The purpose of this position is to function as a Nurse Educator, to provide professional nursing expertise and leadership, implementing, and evaluating care to patients who have diabetes mellitus or are at risk for diabetes. This position is located within the Division of Community Health Services, Health Education Center for Wellness at Shiprock Service Unit, Shiprock, NM. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).

  • Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Provides outpatient education and counseling to individuals and families according to curriculum(s), policies, and procedures of the Health Education Center for Wellness. With each patient, the incumbent assesses individual learning needs, plan, and implement an education plan to meet those needs and reevaluate as needed. Serves as adjunct instructor for weight loss classes, Native Lifestyle Balance program, and other classes that are developed to address patient need. Coordinates diabetes care and patient visits with other departments for prevention of diabetes, diabetes treatment and related complications: Optometry, Dental, Physical Therapy, Pharmacy, and Dieticians. Performs a comprehensive clinical assessment and evaluation of patients with diabetes such as administering medication(s)/ immunizations, taking vital signs, measuring height and weight, point of care testing of blood sugar and/or A1c, and foot exams. Maintains documentation of education and counseling services according to facility and Health Education for Center for Wellness policies. Works as a diabetes education team member to establish goals, objectives, standards of care, and evaluation techniques that contributes to the overall Health Education Center for Wellness planning, policy, and procedure development, program evaluation, and facility performance (QA/PI) and goals set forth in the Special Diabetes Program for Indians project narrative.
