This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
- Total Rewards of a Allied Health Professional This position is responsible for providing services to Veterans on a designated BHIP team and the Urgent Care Clinic within the Behavioral Health Clinic. This professional Social Worker is responsible to provide a variety of services for Veterans in Urgent Care including psychosocial assessments; treatment planning; short-term, solution focused counseling with individuals and families who may be in crisis; education and referral for resources and benefits; case management and post-intervention follow-up. The individual works on a designated BHIP team and the Urgent Care Clinic provides general social work duties to his/her assigned area/team but may be required to work an irregular tour of duty, including weekends, depending on the needs of the service and participate in social work coverage to the medical center off tour. Major duties include but are not limited to: The Social Worker completes psychosocial assessments which are focused on substance abuse, individual and family histories as well as financial, vocational, housing and other related social needs. Works with patients to develop an individualized treatment plan. Participates in discharge/treatment planning with other disciplines and will be responsible for ensuring that discharge/treatment plans are executed in a manner that is timely and appropriate. Provides consultation/education to Veterans and families on community resources, advance directives, and VA benefits. Acts as a consultant to other team/staff members. Provides consultation/education to medical residents on Social Work practice and VA policy and procedure. Provides a full range of social work services within commonly accepted standards of social work practice, which includes casework, individual, family and group counseling, discharge planning, case management and other services as appropriate. Provides wellness/prevention education and facilitate patient/family support groups as appropriate. Coordinates treatment and communicates with other services to facilitate continuity of care for Veterans. The incumbent will participate in committee assignments, as deemed necessary by the service manager. Assists patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death. Will be responsible for maintaining accurate record of relevant trainings and participate in continuing education in accordance with VA policies and state licensure. The Social Worker has the ability to evaluate his/her own practice through professional peer review, case conferences and other organizational means. The Social Worker will complete all mandatory VA trainings in a timely manner. The Social Worker will fulfill ethical responsibilities to Veterans, colleagues and VA in accordance with NASW Code of Ethics, VHA Code of Ethics, and local Ethics policies, which includes but is not limited to compliance with all laws and regulations related to confidentiality and release of information, and protecting Veterans' personal identifying information. Additional duties as assigned. Preferred Experience: Mental health experience, LSW or LCSW. Experience with the geriatric population (educational experience; work experience within a geriatric setting such as a hospital, skilled nursing facility, memory care center; specialty area of study; trainings on older adults).
