About the Position: This position is located at Whiteman Air Force Base, Missouri. This is a Direct Hire Solicitation
- Coordinate and implement treatment plans by arranging for transition to the next level of medical care in compliance with the multidisciplinary treatment plan.
- Conducts therapeutic intervention with individuals, families and/or groups to lessen the impact of acute, chronic and/or terminal illness or injury on the lives of patients and families.
- Provide ongoing risk and case management to evaluate and ensure appropriate levels of service.
- Organize community services on behalf of beneficiaries; and develop and coordinate procedures for the use of community services by related staffs and satellite facilities.
- Educate medical staff on the psychosocial impact of injury, outpatient procedures and community treatment resources.
