Registered Nurse - Nurse Manager (Home Based Primary Care)

The Nurse Manager oversees daily nursing operations in the Geriatrics and Extended Care (GEC) program, ensuring high-quality patient care, staff supervision, and coordination across interdisciplinary teams. • Some duties and responsibilities for the position include but are not limited to: Utilizing problem-solving methodologies to resolve system and/or process problems that impact on patient care or staff performance at the program or service level that is supported by documented evidence of: Assuming responsibility for meeting an identified need Developing and implementing the plan to meet the identified need. Evaluating and providing evidence of improved patient care or staff performance relative to the identified need. Demonstrates role modeling, leadership and advanced clinical expertise to improve the delivery of patient care using the nursing process for patients requiring specialized/complex care. Collaborates with Case Managers, Clinical Specialist, other Nurse Managers, Chief Nurses and the Associate Director for Patient /Nursing Service, Chief of Staff and other services, such as Health Administration Service and all other clinical services in the application of the nursing process to improve outcomes for programs. Utilizes creative problem-solving methodologies to resolve problems that impact patient care or staff performance. Uses a team approach to identify, analyze, and resolve problems. Identifies the need for new or revised policies, procedures, standards, or guidelines and assures compliance. Role models new practice changes to ensure improved patient care. Demonstrates the ability to function effectively in a management role utilizing application of in-depth knowledge, competency, and skills to improve patient care in the area of supervision. Utilizes systems thinking to actively participate in the development and maintenance of the program and delivery of care within an integrated system. Submits reports, to include proficiency, performance appraisals, and incidents that are correct, complete, and reflect current standards, in the correct format and have evaluation for action. Reports submitted within the timeframe required. Major duties of the Nurse Manager/Program Director: Targets veterans with complex chronic diseases that worsen over time and require an interdisciplinary health care team, coordination of care, and the integration of diverse services to cover their complex medical, social, rehabilitative, and behavioral needs. Responsible for the organization, administration, structure, planning, and evaluation of the HBPC Program. Directs the program's clinical services to ensure it follows local and national VHA standards and policies, as well as accreditation standards for home care organizations. Provides comprehensive interdisciplinary care and cost-effective primary care services in the home. Collaborates with other managers/administration to initiate improvements in care and delivery and to identify and resolve problems affecting client care and program operations. Functions as a role model, educator, and manager and provides leadership to an interdisciplinary team.
