The Findlay VA Clinic is seeking a Full-Time Registered Nurse (RN) - Outpatient Float, aligned under Ambulatory Care Services.

• The Ambulatory Care Float Team- Staff Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific care to primary care patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. This nurse provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Demonstrates leadership in delivering and improving holistic, primary care through collaborative strategies with others. Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional outpatient nursing perspective in discussions with the interdisciplinary team. Partners with others to effect change and produce optimal outcomes. Supports colleagues through knowledge sharing to provide safe, quality outpatient nursing care. Shares educational findings, experiences, and ideas with peers. Questions PACT clinical practices for the purpose of providing evidence-based care. Participates in activities and strategies to sustain an evidence-based-practice culture. Responsibility of Nurse : The nurse is organizationally a staff nurse aligned under Ambulatory Care Services and will be assigned to various Primary Acute Care Teams (PACT) and/or Sub-Specialty Clinics. To support overall health care delivery efforts of the VA Ann Arbor Healthcare System (VAAAHS), the RN may be assigned to one or more clinical areas within the medical center, a community-based outpatient clinic (CBOC), and/or offsite ancillary clinic to assume critical clinical nursing roles ranging in complexity and scope. All of the areas/clinics/patient populations the RN incumbent would be assigned to range in overall acuity, requiring the RN to exhibit adaptability within their clinical practice. An annual clinical competence review is conducted by a designated nurse peer, nurse educator, nurse supervisor, and/or nurse practitioner. The nurse performs his/her duties independently on a day-to-day basis. Refer to the Organizational Chart for the supervisor chain of command.
