About the Position: This is a Nurse (Clinical/Case Manager) position at Fort Drum, New York. This is a Direct Hire Solicitation. Salary negotiation is available for those new to Federal employment. Recruitment, Relocation and/or Student Loan Repayment may be authorized.

• Serve as Nurse Case Manager responsible for analyzing problems associated with providing health care to patients and/or beneficiaries for the Military Treatment Facility (MTF).
• Conduct comprehensive assessments of beneficiary’s health needs to develop a plan of care.
• Educate both patient and members of the healthcare delivery team about case management, healthcare and treatment options, community resources, insurance benefits, psychosocial concerns, etc. so that informed decisions can be made.
• Consult and coordinates with other multi-disciplinary care professionals, agencies, organizations and other ancillary support systems.
• Serve as a liaison between the patent and the health care professionals.
