The Registered Nurse - PACT Outpatient, demonstrates integration of cognitive skills and technically competent practice while providing care to clients. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Influences care outcomes by collaborating with members of the interdisciplinary team.

• Registered Nurse - Women's Health PACT Outpatient Focuses on foundational, individual nursing practice for an all Women's PACT Team. Provides direct female Veteran care to include assisting with female related procedures such as PAP smear, coordinates care within Primary Care, manages assigned panels and delivers health promotion of chronic and complex female disease management. The RN will work collaboratively in an interdisciplinary team to manage panels and provide health education of chronic and complex care management. Additionally, the RN will apply nursing expertise , evidence-based direct care when assessing and triaging unscheduled walk-ins via face to face, over the phone, video or secure messaging. Using all formats of electronic communication such as Message manager, secured messages, JLV the RN will foster an open communication with all stakeholders and veteran. Contributes to nursing education, orientation, competencies, and quality improvement. Demonstrates leadership through collaboration, sharing knowledge, evaluating practice, fostering a supportive environment, and promoting evidence-based practice within the interdisciplinary team. Demonstrates leadership and independent decision-making in managing complex primary care. Leads the development and evaluation of policies, procedures, and guidelines. Influences practice and outcomes at the program or service level, evaluates evidence-based practice results for individuals, groups, and populations, and contributes to organizational-wide improvements in primary care.
