This position is located at New Mexico VA Health Care System (NMVAHCS). The Clinical Pharmacist Practitioner (CPP) serves as a clinical provider and education leader within and beyond Pharmacy Service. This role combines advanced patient care in Academic PACT, Weight Management, and other specialty clinics with responsibility for coordinating, developing, and delivering pharmacy and medication-related education programs.
- Duties include but not limited to: Assessing, designing, initiating, modifying and monitoring therapeutic drug plans to achieve patient centric outcomes. Perform pertinent physical assessment as needed. Obtain medication histories from patients and summarizes significant findings in the medical record and/or to the provider as appropriate. Document clinical interventions in CPRS in a timely and professional manner. Participate in research studies as assigned/requested. Serve on medical center and/or VISN committees. Monitors for and reports drug errors, adverse drug reactions, allergies and patient compliance issues. Develop and maintain evidence-based training materials and competency tools for pharmacists, technicians, and learners. Provide interprofessional education, including new nurse/provider orientations, and serve as an educational resource across service lines. Coordinate orientation and ongoing training for new employees, students, and volunteers; precept pharmacy students and residents. Maintain faculty appointment with UNM College of Pharmacy and support related academic and trainee activities.
