Greater Los Angeles Healthcare System seeks a board-certified Pharmacy Technician (CPhT) for the Clinical Pharmacy Service Line. The incumbent will assist with patient care consults and support Clinical Pharmacist Practitioners (CPP) in disease state management clinics, helping address medication needs and reducing administrative tasks for CPPs. Responsibilities include population health management, optimizing medication access, and improving efficiency among patients, CPPs, and clinical staff.

  • Duties include but are not limited to: Review medication profiles for prescription/therapeutic duplication, noncompliance, and allergies/adverse drug reactions; report to CPP. Analyze clinical dashboards and lab reports to identify patients needing intervention; notify CPP. Provide education to patients on operational issues and therapy compliance. Contact patients for lab, appointment, and other clinical reminders. Inform patients of lab results and CPP recommendations. Conduct initial medication reconciliation for new clinic patients prior to visits. Perform routine patient follow-up via phone to assess medication compliance, allergies, adverse events, and other health factors. Schedule appointments for CPPs (in-person, telephone, video). Triage patient consults and messages to appropriate specialty or staff. Accurately document patient interactions in CPRS using templated notes. Enter clinic and lab orders with CPP signature as required. Assist CPPs with data collection, compiling reports, and presenting findings. Screen refill requests, ensuring compliance with controlled substance protocols (urine drug screens, naloxone, STORM, PDMP). Provide naloxone education and increase patient access to overdose prevention. Assist with medication inventory, Omnicell maintenance, and drug shortage monitoring in ambulatory clinics. Assist with population health projects, outreach programs and clinical reviews of high-risk patients. Assist the CPP with other operational or administrative tasks not listed above. Assist collaborating CPPs in related specialty areas at the discretion of the Associate Chief, Clinical.
