About the Position : This position is located at Aberdeen Proving Ground, Maryland at Kirk U.S. Army Health Clinic. Incumbent will be providing social work services at Patient Centered Medical Home. Hours : Monday- Friday 0730-1630 This is a Direct Hire Authority (DHA) solicitation utilizing the DHA for Certain Personnel of the DoD to recruit and appoint qualified candidates to positions in the competitive service.
- Provide targeted assessment and evaluation of presenting problem.
- Formulate evidence-based interventions for a wide range of mental health conditions.
- Collect bio-psycho-social information about patients.
- Document care and recommendations concisely in the medical record chart.
- Refer patients to Specialty Mental Health Care as needed.
