Community Care Physicians jobs

Community Care Physicians started in 1984 as a physician-led group and has grown into an independent multispecialty medical group serving New York's Capital Region. Independent means exactly what it says here: the group belongs to no hospital system and no insurance company, and it answers to neither. It's still physician led in a specific way, too. The doctors who run the organization keep practicing in their own specialties while they do it, and a central company, CCP TASS, handles all the business functions so the individual practices don't have to.
The numbers give you the shape of it: about 1,800 employees, more than 420 practitioners, and over 30 medical specialties across 70 practices in Albany, Rensselaer, Saratoga, Schenectady, Montgomery, and Warren counties, covering primary care, specialty care, urgent care, and wellness services. The offices are connected through one shared electronic health record, so a patient's doctors work from the same chart across practices, and patients carry one portal, mycareDOT, across the whole group. CCP describes its model as patient centered care built on mutual trust, with the patient named as the leader of the care team. For a clinician, the practical read is referral partners across 30 specialties inside one independent group, after more than 40 years of running that way.
