In addition, the incumbent provides geriatric and/or palliative coordination and oversight for the CLC. Duties includes but limited too: - Coordinates comprehensive, interdisciplinary, medical care and case management in CLC for Veterans with complex medical, social and behavioral conditions. - Acts as a collaborating physician for APP providers in the CLC. - Oversees admissions to the program. - Provides consultation to the medical staff for patients with complex treatment or care needs in the home. - Collaborates in the monitoring of quality and safety concerns within the program. - Communicates relevant responsibilities, policies and procedures to providers. - Participate in staffing/selection of CLC providers. - Serves as the Supervisor for the CLC providers. - Oversees strategic planning for the CLC/RECC ICC. - Monitors performance of medical care provided by Physicians and providers in the CLC. - Participates in the CLC related educational programs including provision of in-services, as needed and appropriate. - Ensures that the CLC providers are current with educational requirements for licensure and are current with VA educational requirements. - Participates in the establishment of policies, procedures, and guidelines necessary for clinical care in collaboration with other members of the interdisciplinary team. Work Schedule: Monday-Friday 8:00am-4:30pm
