The Community Living Center Admissions Coordinator, Registered Nurse, assists with patient planning assessments, identification of barriers, and activities required to successfully transition the patient into the CLC setting.
- The Community Living Center Admissions Coordinator, Registered Nurse, duties include, but are not limited to the following: Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Collaborates with the interdisciplinary team to identify veterans who are appropriate for admission to the Community Living Center (CLC). Focuses on individual growth and development in practice with increased levels of self-direction. Possess a keen understanding of the admission criteria of each CLC program area. Collaborates with the members of the healthcare system and stakeholders to provide a seamless, timely referral system to Veterans seeking admission to the CLC. Collaborates with both referring and accepting staff ensuring that Veterans are placed in the program area that can best meet their needs. Collects and maintain data that will enhance program planning and placement. Initiates consultation for complex administrative and clinical issues.
