Ute Pass Regional Health Service District (UPRHSD) is seeking a part-time Advanced Practice Registered Nurse (APRN) to support its Mobile Integrated Health (MIH) programs in Woodland Park, Colorado and across the District’s four-county service area. This grant-funded, limited-term position provides primary, urgent, preventive, and follow-up care in patients’ homes and community settings, helping patients receive appropriate care where and when they need it while reducing unnecessary emergency department visits and hospital readmissions.

The APRN will work collaboratively with the Medical Director, Community Paramedics, District leadership, healthcare providers, and community partners to assess patients, develop and update care plans, order and interpret diagnostic tests, prescribe medications and treatments within scope of practice, support chronic disease management, educate patients and families, and contribute to quality assurance and program reporting for MIH initiatives.The ideal candidate is a clinically strong, compassionate, and highly adaptable APRN who is comfortable providing care outside of a traditional clinic setting. This person will bring sound judgment, excellent communication skills, and the ability to build trust with patients and families while collaborating effectively with Community Paramedics, EMS personnel, physicians, and community partners.

Minimum Qualifications
  • Active, unencumbered Advanced Practice Registered Nurse (APRN) license in the State of Colorado, or eligibility to obtain licensure within thirty (30) days of hire. 
  • Master's degree (or higher) in an advanced practice nursing specialty.
  • National certification as a Nurse Practitioner with advanced training in one of the following: Family Practice, Internal Medicine, Emergency Medicine, or Geriatric Medicine.
  • Current DEA registration, or eligibility to obtain within thirty (30) days of hire, for prescriptive authority.
  • Current AHA BLS, ACLS, and PALS certification or ability to obtain.
  • Valid driver's license to travel throughout the District's MIH service area to conduct home and community visits.  
  • Completion of a District-approved Emergency Vehicle Operator Course (EVOC) within thirty (30) days of hire.
  • Strong clinical assessment, critical-thinking, and documentation skills with the ability to work in austere environments with limited communications access. 
  • Ability to work both independently and collaboratively within a multidisciplinary team, including Community Paramedics, EMS field staff, and administrative personnel.
  • Motivational Interviewing (MI) or ability to obtain within one (1) year of hire.

Preferred Qualifications
  • Experience in Emergency Medical Services (EMS) community paramedicine, MIH, Mobile Crisis Response (MCR) and home-based primary care, or geriatric care management.
  • Experience working within EMS, rural health, or public safety organizations.
  • Familiarity with Colorado special district or public agency operations.
  • Bilingual or multicultural community experience relevant to the District's service area.
Additional Information
  • Schedule: The schedule is approximately twenty (20) hours per week, with specific days and hours coordinated with Operations leadership based on program and patient needs.
  • Grant-Funded Position: This is a grant-funded, limited-term position anticipated to continue for up to five (5) years. Continued employment is contingent upon the availability of grant funding and ongoing program and operational needs. The position may end earlier if grant funding is reduced, discontinued, or otherwise becomes unavailable.
