This Nurse Practitioner (NP) will be practicing in the Rehabilitation, Extended and Community Care (RECC) Division, Community Living Center (CLC) Service. They provide care and/or services appropriate to the age of the patients being served, taking into consideration age-related differences of the various Veteran populations, and which is appropriate to the cognitive; emotional, and chronological maturation of the patient.

  • THIS IS NOT A REMOTE POSITION Independently manage medically complex adult and geriatric patients with multiple acute and chronic health conditions and developing individualized, evidence-based plans of care. Recognize and manage changes in clinical condition, prioritize competing health concerns, and adjust treatment plans to optimize patient outcomes. Prescribe and monitor pharmacologic and non-pharmacologic interventions within scope of practice, including ongoing evaluation of treatment effectiveness and patient response. Coordinate specialty referrals, consultations, transitions of care, and follow-up for patients with complex or evolving healthcare needs. Collaborate closely with physicians, nursing staff, social workers, pharmacists, rehabilitation therapists, dietitians, mental health professionals, and other members of an interdisciplinary healthcare team. Lead and actively contribute to interdisciplinary care planning, team-based clinical decision-making, and initiatives designed to improve continuity, quality, and timeliness of care. This position must have a strong knowledge of age-related changes and the clinical, functional, cognitive, and safety needs of older adults, with an emphasis on maintaining function, independence, and quality of life.
