This position is in the HUD-VASH program, providing in home care to Veterans. The RN possesses thorough knowledge of utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines. This position is outpatient but will require collaboration with the interdisciplinary team, along with the Veteran, their family/caregiver, and internal and community-based services involved in providing care to the patient
- - Leads professional practice using the nursing process (assessment, diagnosis, outcome identification, planning, implementation and evaluation) to improve evidence-based and/or data-driven outcomes for an identified patient population and/or specialized areas within immediate practice setting. - Improves Veteran/patient driven care by optimizing the patient experience, satisfaction and/or safety for the immediate practice setting. - Communicates and collaborates as an effective leader of the interprofessional team, at the expert level, with improved evidence-based and/or data-driven outcomes in the immediate practice setting. - Expands knowledge base and practice evaluation through mentoring, precepting and/or teaching to improve outcomes in the immediate practice setting. - Leads the integration of evidence-based practice/research with clinical expertise to improve quality of care and resource utilization with improved outcomes for the immediate practice setting. - Utilizes technical and manual skills and coordinates with the social work case managers. - Perform a wide range of patient care and support duties both on-site and in the community. - Assess and refer medically fragile and vulnerable Veterans and consulting with Team Leads and/or Supervisor. - Provides education regarding chronic health conditions and disease management, such as diabetes and hypertension. - Provides education on lifestyle modifications, nutrition, and medication side effects. - Works with a Veteran's PACT Team to discuss treatment plans, consults, and requests for durable medical equipment (DME). - Answers treatment, procedure or disease related questions, as appropriate. - Provides support and consultation to all HUD-VASH staff.
