This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.   • Functions or Scope of Assigned Duties include but are not limited to the following: Comprehensive Assessment and Follow-Up: The CM conducts thorough assessments to understand the Veteran's needs, concerns, and goals. This includes reviewing military history, social supports, housing, transportation, education, employment, finances, and care-planning needs. Reassessments are completed as needed and in collaboration with the Veteran, their family or caregiver, and the care team. All documentation is entered into the EHR. Goal Setting and Resource Identification: The CM helps Veterans and their families clarify priorities and identify supports, benefits, and services-both within the VA and in the community. This includes exploring natural supports and the Veteran's own strengths and coping skills. Care Planning, Implementation, and Communication: The CM develops and carries out care plans based on the Veteran's needs, coordinating with VA providers, community partners, and caregivers. They assist with navigating systems, connecting Veterans with health and mental health services, education programs, self-management tools, and social supports. Communication is proactive, patient-centered, and focused on keeping everyone informed throughout the process. Therapeutic Engagement and Psychosocial Support: The CM builds strong, supportive relationships with Veterans, families, and caregivers using recovery-oriented, strengths-based approaches such as Motivational Interviewing, Solution-Focused techniques, Health Coaching, and Shared Decision Making. When providing psychotherapy, the CM completes appropriate screenings, uses measurement-based care, and develops treatment plans. They also offer crisis intervention and help coordinate higher levels of care when needed. Care Coordination, Referrals, and Transitions: The CM ensures Veterans receive timely, appropriate referrals and collaborates closely with VA programs in Eugene and across VISN 20, including PACT, Whole Health, specialty mental health, substance use disorder programs, Suicide Prevention, Homeless Programs, and more. They help manage transitions between VHA, DoD, federal, state, and local services-including community care-so Veterans move smoothly between levels of support. Program and Outcome Evaluation: The CM uses established case management tools and processes to track progress, monitor quality, and support program evaluation. Documentation: All documentation is completed accurately, promptly, and in accordance with VHA, Joint Commission, Medical Center, and service-level requirements. Professional Responsibilities: The CM participates in peer review, team and clinic meetings, and ongoing professional development to maintain licensure and certifications (including BLS). Productivity and quality expectations align with the complexity of cases and professional standards. The CM may also join committees or work groups as needed.
