The VA Pacific Islands Health Care System (VAPIHCS) is a level 3 facility that provides access to patient care to over 130,000 veterans across a 2.6M square mile area of the Pacific Ocean delivering a full range of comprehensive health care services. The VAPIHCS operates eight community-based outpatient clinics (CBOCs) covering seven county areas. These outpatient clinics are in America Samoa, Guam, Hilo, Kauai, Kona, Leeward, and Maui.

• Responsible and accountable for all elements of the nursing process, influences outcomes by collaborating with members of the interdisciplinary team, and executes position responsibilities that demonstrates leadership, experience, and creative approaches, and systematically evaluates current practice. Demonstrates the following core informatics functions to include content management, application support, health information technology system optimization, enterprise data management, business architecture management, partnership management, requirements management, and change management. The Nurse III Informaticist Registered Nurse (RN) exercises leadership in the assessment, planning, implementation and evaluation of hardware and software applications integrated with the electronic health record (EHR) and associated programs/applications. The Informaticist collaborates with interdisciplinary team members, including other program and service leaders at the facility and network. They work closely to establish and maintain effective relationships with all levels of Veterans Affairs (VA) staff nationwide. The Informaticist is responsible for the documented outcomes at the program or service level. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility, Veterans Integrated Service Network (VISN) or Veterans Affairs Central Office (VACO), illustrating performance and leadership that benefit the facility. Dimensions of Nursing Practice: PRACTICE: Demonstrates expertise in professional practice using process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) with improves evidence-based and/or data-driven outcomes beyond the immediate practice setting. Expectations: Applies advanced clinical knowledge/judgement to lead staff involvement in planning, decision-making, and evaluating outcomes beyond the immediate practice setting. Develops recommendations and strategies to address and resolve complex issues and problems, providing direction for all key stakeholders . Establishes formal and informal consultative relationships that can also provide professional development and mentorship opportunities. Leads and/or supports the use of clinical guidelines, into practice, information system and informatics solutions, and knowledge bases. Uses professional standards, scientific evidence, and informatics practice to evaluate programs and/or services activities. VETERAN/PATIENT DRIVEN CARE: Leads initiatives to improve Veteran/patient experience, satisfaction and/or safety beyond the immediate practice setting. Expectations: Leads initiatives to improve the Veteran experience i.e., care coordination, timeliness to resolve issues, negative trends, or environment of care concerns, beyond the immediate practice setting. Demonstrates leadership in patient advocacy by protecting health, safety, and rights of patient population groups beyond the immediate practice setting. Leads staff to identify, analyze and address ethical issues that impact patients, caregivers, and staff beyond the immediate practice setting. Models high reliability principles and I CARE values to improve the consistency of care delivery and patient outcomes beyond the immediate practice setting. LEADERSHIP: Communicates and collaborates as an effective leader of teh interprofessional team, at the expert level, with improved evidence-based and/or data driven outcomes beyond the immediate practice setting. PROFEESIONAL DEVELOPMENT: Enhances knowledge base and practice evaluation, at the expert level, with improved outcomes beyond the immediate practice setting. EVIDENCED-BASED PRACTICE/RESEARCH: Leads the identification and integration of evidence-based practice/research with clinical expertise to improve quality of care and resource utilization at the expert level with improved outcomes beyond the immediate practice setting.
