The Referral Coordination Registered Nurse is responsible and accountable for all elements of the nursing process when providing direct patient care. Assesses, plans, implements, and evaluates the effectiveness of the Veteran's care. Serves to support delivery of high quality, timely care to the Veteran receiving specialty care services. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- Referral Coordination Team (RCT) Registered Nurses streamline receipt, triage, and coordination of referrals to ensure timely, appropriate care. They guide Veterans through internal VA and community care options, applying referral criteria, documentation standards, utilization management principles, and VHA guidelines. RCT RNs act as liaisons between ordering providers, specialty services, community care, and the Office of Integrated Veteran Care to support access, ensure required pre-work is completed, and maintain compliance with VA referral systems. They use standardized triage tools, support scheduling, and uphold key performance indicators. Applies principles and basic critical thinking with a focus on growth and self-direction. Manages referral triage, care option guidance, and coordination with specialty and community services. Ensures referrals meet requirements and Veterans receive timely, informed care Demonstrates proficient clinical decision-making and deliberate planning. Provides informal, constructive feedback and helps foster a supportive environment. Shares knowledge, participates in interdisciplinary discussions, and contributes to evidence-based practice. Questions practice to promote evidence-based improvements and supports a culture of EBP. Uses advanced critical thinking and evaluates outcomes of practice changes at program or service levels. Responsible for complex, documented organizational outcomes. Supports professional development, shares knowledge, and ensures evidence-based decisions are evaluated for individuals, groups, and populations. Supervisory ControlsAll levels report to designated rating/approving supervisors and are accountable to the Associate Director of Patient Care Services (ADPCS).
