Registered Nurse - Primary Care/Pain Management

Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.

• Major Duties include, but are not limited to: Provide direct nursing care and case management for Veterans with chronic and complex pain conditions within the primary care setting. Collaborate with physicians, nurse practitioners, pharmacists, psychologists, and other interdisciplinary team members to develop and implement individualized pain management care plans. Conduct comprehensive pain assessments and monitor treatment efficacy and side effects. Support safe opioid prescribing practices, including monitoring compliance with Opioid Safety Initiative (OSI) guidelines, informed consent processes, and Prescription Drug Monitoring Program (PDMP) checks. Educate Veterans and caregivers on pain management options, including non-pharmacological modalities (e.g., Whole Health, mindfulness, physical therapy referrals, acupuncture) and safe medication use. Coordinate referrals to specialty pain clinics, behavioral health, physical medicine and rehabilitation, and complementary/integrative health services. Monitor high-risk patients for signs of substance use disorder, diversion, or adverse outcomes, and escalate concerns per protocol. Document care thoroughly and accurately in the electronic health record (CPRS/Cerner) in compliance with VA and Joint Commission standards. Participate in quality improvement initiatives related to pain management outcomes and opioid safety metrics. Provide telephone triage and telehealth support for pain-related patient concerns.
