- The Specialty Clinic Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex specialty care. This nurse provides patient care, care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for specialty care outpatients. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, quality specialty nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented outcomes at the specialty care program or service level. Specialty care program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility. The RN operates within a dynamic clinical environment that requires consistent physical presence to facilitate timely care coordination, direct patient assessment, and real-time collaboration with the interdisciplinary team, and must demonstrate adaptability and professional flexibility in meeting fluctuating patient volumes, staffing needs, and organizational priorities. The RN routinely communicates with Licensed Independent Providers (LIPs), RNs, and allied health professionals using sound interpersonal skills, maintains awareness of sensitive information at all times, and works independently based on established guidelines, precedents, and regulations with minimal supervision required. Performance is evaluated on the overall effectiveness of operations, accuracy and adequacy of work, timeliness, and conformance to established policies and procedures. The RN maintains a strong, current knowledge base relevant to the assigned specialty area and demonstrates competency in specialty-specific clinical skills as documented in the applicable competency check-off for the clinic. Key Responsibilities - Participate in the patient check-in process and provide direct patient care to all patients in the Specialty Clinics - Serve as a clinical resource for Licensed Practical Nurse (LPN), Certified Nursing Assistant (CNA), and Health Technician staff, and assist with clinic flow - Assess patients presenting with clinical changes and apply the nursing process in an ongoing, systematic manner from initial contact through the entire clinic encounter - Perform minor procedure setup and conduct pre-procedure and triage telephone assessments - Assist with ordering necessary clinic supplies - Perform focused case management of designated patient populations as appropriate to the specialty area - Communicate and collaborate with the interdisciplinary team - including LIPs, Pharmacy, and allied health professionals - to develop and coordinate individualized Veteran treatment plans - Review and manage incoming consults, including routine scheduling, completion of econsults, or discontinuation as appropriate. For RNs not independently managing a casemanagement panel, this is done in coordination with the Nurse Case Manager; for RNs who independently perform case management for an assigned patient panel, this is done as part of their responsibilities - Either perform ongoing case management; including consult review, care-coordination follow-up, and related monitoring - independently for an assigned patient panel, consistent with individual competency, training, and scope of practice, or provide basic case-management coverage - including consult review and routine care-coordination follow-up - during short-term absence of the Nurse Case Manager, consistent with competency and scope of practice ***Continued in the Education Section.
