Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines.

  • The Clinical Provider Liaison practices as a registered nurse in the Office of Community Care (OCC) Program Department and is qualified by education and experience to provide direct staff leadership support to service chiefs and others in planning, designing, integrating, implementing, modifying and evaluating the effectiveness of program components and/or services to improve the quality of patient care, the appropriateness and timeliness of services provided, assist in patient flow/reduction of length of stay. The Clinical Provider Liaison serves as a leader, utilizing sound clinical judgment and established criteria for guidance in developing, implementing, and maintaining the Community Outreach Program; provides guidance in ensuring compliance with the use of clinical review tool, Joint Commission, and other regulatory requirements; demonstrates leadership in data management and analysis and in the use of data and information to identify opportunities for improvement in outcomes; and communicates effectively with all providers of care, ancillary and support services, and external customers to facilitate efficient patient-centered care and patient flow. This position will also be responsible for the following duties but not limited to: Traveling in the community to establish provider agreements with Community Partners. Ensuring seamless integration of intersecting services for Community Partners Monitor and track completion of provider agreements, updated annual compliance as updated in the Mission Act Program standard operating procedure Acting as a liaison for all Community Partners Ensuring Community Outreach measures are met and assist with development of strategies to improve performance. The Clinical Provider Liaison shall provide Registered Nurse services for the support of OCC program for the Chief of Staff and the Office of the ADPCS, VHSO. Scope of duties includes direct patient care services, consultation, and participation in Mission Act education programs, and providing ongoing care for an enrolled population. Contributing to the environment of care through maintenance of a safe and supportive environment for patients, staff, and other individuals served by or providing services within the organization. The Clinical Provider Liaison will follow all standard operating procedures and will participate in educational and quality improvement programs within their assigned clinical area.
