As a professional social worker in the Primary Care Clinic and surrounding community-based outpatient clinics, you will provide clinical social work services, assess and treat complicated psychosocial problems, conduct crisis intervention, and offer short-term counseling. You will collaborate with interdisciplinary teams, manage cases, coordinate community placements, and act as a liaison to VA and community resources to help Veterans achieve maximum independence.

  • VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Major Duties, but not limited too: Independently complete thorough psychosocial assessments to determine patient functioning and needs, and provide short-term, solution-focused counseling to Veterans and families experiencing crisis or complex medical, psychiatric, and emotional problems. Actively participate as an integral member of the interdisciplinary treatment team to develop, implement, and coordinate collaborative treatment goals and interventions. Provide comprehensive case management services across the continuum of care, utilizing knowledge of human growth, development, and life span issues to help Veterans cope with illness, disability, loss, and grief. Facilitate timely community placements (such as contract nursing homes, hospices, and residential care) and serve as a vital liaison connecting Veterans and their families to VA benefits, specialty programs, and community resources. Provide consultation and education to treatment team members, Veterans, and families regarding psychosocial needs, community resources, advance directives, and supportive educational classes. Maintain accurate productivity and quality standards, support facility and VHA missions, utilize VHA software applications, and provide cross-coverage or assistance within the Social Service department and CBOCs as needed. Work Schedule: Monday - Friday, 7:30am - 4:00pm (Adhoc eligible) Functional Statement #: 40024F Recruitment/Relocation Incentives: Authorized. Permanent Change of Station (Relocation Assistance): Not authorized. Pay: Competitive salary and regular salary increases. Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Telework: Not available.
