This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.   • The Registered Nurse in the Minimum Data Set (MDS) program is responsible for assessing, planning, implementing, coordinating, and evaluating patient care across the continuum of care. The nurse promotes patient education, self-management, safety, and customer satisfaction while collaborating with the interdisciplinary healthcare team to improve patient outcomes. As nurses progress from Nurse I to Nurse III, they demonstrate increasing levels of clinical expertise, leadership, independent decision-making, mentoring, quality improvement, and program-level responsibility. The position requires knowledge of long-term care standards, evidence-based practice, management of acute and chronic illnesses, and palliative and hospice care, while ensuring compliance with Veterans Health Administration, CMS, and Joint Commission requirements. Key Duties and Responsibilities Assess, plan, implement, coordinate, and evaluate individualized patient care. Administer medications, treatments, and procedures according to established policies and guidelines. Coordinate care, patient education, and self-management activities to support positive health outcomes. Collaborate with interdisciplinary teams to ensure safe, effective, and patient-centered care. Apply evidence-based practice and participate in quality improvement and performance improvement initiatives. Advocate for patient safety, rights, satisfaction, and ethical nursing practice. Support professional development through continuing education, mentoring, and knowledge sharing. Lead and contribute to program, unit, or service-level initiatives that improve patient care outcomes and nursing practice. Maintain compliance with VA policies, long-term care regulations, CMS requirements, and Joint Commission standards. Foster a culture of teamwork, accountability, leadership, and continuous improvement in patient care delivery.
