- VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4 Total Rewards of a Allied Health Professional Duties include, but are not limited to: Conducts independent home visits for Veterans with complex medical and psychosocial needs, delivering direct nursing care and providing education to patients and caregivers in alignment with the individualized care plan established by the RN Case Manager. Collaborates with the Home Based Primary Care (HBPC) Interdisciplinary Team including the Physician Preceptor, Registered Nurse, Licensed Practical Nurse, Occupational Therapist/Physical Therapist, Social Work, Dietitian, Speech Therapist, Psychologist, Pharmacist and Advanced Practice Provider. Collaborates with services internal and external to the VA to manage the care needs of the patients. Provides patient and family health education with a focus on self-management, prevention, health promotion and wellness based on the patient's individual goals. Supports additional teams by completing home visits as needed based on patient care demands. Demonstrates strong prioritization skills in determining the order of patient visits according to acuity and need. Utilizes Home Based Primary Care (HBPC) policies and procedures to provide autonomous, high-quality care in the home setting. Possesses the knowledge and skills to carry out more specialized patient care activities. Knowledge of program regulations and is able to carry out objectives of the Nursing program. Contributes to the development of the plan of care for assigned veterans outlining program modalities and updates per policy and as changes occur.
