This position is eligible for the Education Debt Reduction Program (EDRP) Join VA Black Hills Health Care System as a Community Living Center Restorative Coordinator Registered Nurse and help Veterans maximize independence and function. Lead restorative nursing care, assess and enroll Veterans, develop measurable goals, monitor progress, collaborate with therapy teams, and strengthen staff practice through education and mentoring.
- Make a lasting impact on Veterans' independence and quality of life as a Community Living Center Restorative Coordinator Registered Nurse. In this specialized role, you will help Veterans maintain or regain function by connecting individualized restorative goals with coordinated nursing and therapy services. You will work closely with Veterans, families, nursing staff, Occupational Therapy, Physical Therapy, educators, and the interdisciplinary team to guide restorative care from admission through ongoing evaluation. This position also offers the opportunity to serve as a clinical resource, mentor staff, strengthen restorative nursing expertise, and use measurable outcomes to advance the quality and effectiveness of the program. Duties include, but are not limited to: Assess Veterans for restorative care needs, complete initial screenings, and determine eligibility for admission to the Restorative Care Program. Develop and implement measurable functional goals designed to help Veterans maximize independence and achieve their highest practical level of functioning. Collaborate with Occupational Therapy, Physical Therapy, nursing educators, Veterans, families, and the interdisciplinary team to coordinate individualized restorative care plans. Evaluate Veteran progress at least monthly and adjust restorative interventions, goals, therapies, and activities in response to changing needs and outcomes. Complete restorative nursing components of the Minimum Data Set, including observation periods, quarterly reassessments, and significant changes in care. Monitor restorative staff assignments to ensure care responsibilities align with demonstrated education, training, and clinical capabilities. Mentor, coach, and educate restorative nursing staff while developing in-services and learning opportunities that build clinical skills and documented competencies. Evaluate program outcomes using Minimum Data Set quality indicators and implement restorative nursing strategies that support effective, evidence-based care and improved Veteran outcomes.
