The Community Health Registered Nurse serves as liaison for Veterans and families between external agencies and the VACHS. They participate in the coordination of the delivery of community-based care services, such as contract nursing homes and purchased skilled home care services.

  • Duties include but are not limited to: Interact with agencies that provide non-institutional care to enrolled Veterans to ensure timely evaluation and receipt of the plan of care and appropriate documentation of the service provided. Act as the liaison in coordination of seamless discharges from inpatient VA and Non-VA hospitals and transition back to VA Primary Care; and provide leadership in meeting the non-institutional care needs of Veterans, families and community agencies in support of our VA mission. Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.
