Registered Nurse - Community Care Coordinator

The Community Care Coordinator RN streamlines receiving, triaging, and coordinating timely care. The nurse applies strong clinical judgment, planning, and critical thinking; understands referral processes, documentation, and VHA standards; and guides Veterans through VA and community care options while serving as a liaison to ensure timely, high-quality care.

• The Community Care Coordinator works collaboratively within the Office of Integrated Veteran Care (IVC) to triage Requests for Services (RFS) and coordinate care processes outlined in the IVC Field Guidebook. Responsibilities include developing care coordination plans, reviewing medical documentation from community providers, communicating care needs, and supporting Veterans through the full episode of care. The CC Coordinator influences outcomes by partnering with interdisciplinary team members across VA and community settings. The CC Coordinator follows VA-mandated processes and uses approved health information systems to assess, document, exchange, and track care coordination activities. They apply delegation of authority, when applicable, to perform at the highest level of their role, aligning high-quality care with key performance indicators and practice standards The Coordinator fosters a safe, supportive environment that promotes professional development, contributes nursing insights to interdisciplinary discussions, and collaborates to drive positive change and optimal outcomes. They support colleagues through knowledge sharing, encourage evidence-based practice, question clinical processes to promote improvement, and participate in strategies that sustain an evidence-based care approach.
