Responsible and accountable for coordinating timely, Veteran-centered care for Veterans receiving services in the community. Uses the nursing process to assess care needs, support referrals and follow-up, provide patient education, and promote continuity of care throughout the community-care episode. Collaborates with Veterans, caregivers, VA staff, community providers, and the interdisciplinary team to support high-quality care, timely access, and customer satisfaction.
- Major Duties include but are not limited to: Assesses Veteran needs and develops care-coordination plans with Veterans, caregivers, VA providers, community providers, and interdisciplinary team members. Educates Veterans and caregivers regarding care options, self-management, referral processes, and follow-up expectations. Coordinates referrals, appointments, clinical documentation, follow-up care, and communication between VA and community providers. Documents and tracks care-coordination activities using VA health-information systems while protecting sensitive information. Promotes timely access, continuity of care, patient safety, Veteran satisfaction, evidence-based practice, and quality improvement. Reviews community-provider medical records and supports clinical-review, utilization-management, documentation, and regulatory requirements. Triages and coordinates community-care Requests for Services and supports Veterans through the full referral and episode-of-care process. Performs assigned nursing duties under appropriate supervision and maintains required competencies, training, and professional development.
