The Miami VA Healthcare System (Bruce W. Carter VA Medical Center)Social Work Service is seeking an experienced and dynamic Senior Social Worker-Home Based Primary Care. The position supports the Veteran Directed Care (VDC) program, which provides patient-centered, non-institutional long-term care options for Veterans in home and community settings Duties and responsibilities include, but are not limited to: Oversee the VDC program's administrative, technical, and budgetary functions, ensuring compliance with VA guidelines, standards, and performance measures Provide direct patient care when applicable; supervise and mentor clinical social work staff; ensure high-quality, equitable care for Veterans Work with healthcare teams, behavioral health professionals, and other stakeholders to integrate social work services into comprehensive Veteran care Manage individual and group cases, coordinate referrals to community and government agencies, and advocate for Veteran needs Supervise social service staff, provide coaching, and support professional development Develop and implement clinical program policies, procedures, and service standards Monitor program effectiveness, identify inefficiencies, and recommend improvements Ensure adherence to VA and community care standards; prepare and submit program reports to VA Central Office and VISN leadership Manage contracts with community agencies, including negotiation and compliance Build and maintain relationships with Veterans, caregivers, local/state/federal agencies, and non-governmental organizations Work Schedule: Monday - Friday, 8:00 am - 4:30 pm(Note: Work schedules are determined and approved by the supervisor. The incumbent may be required to work various shifts on a permanent or temporary basis based on facility/patient care needs).
