About the Position: This position is located at Army Health Clinic Munson at Fort Leavenworth, Kansas. This is a Direct Hire Solicitation Salary negotiation may be available for those candidates who are new to Federal service. A recruitment or relocation incentive may be authorized. Student Loan repayment may be authorized. Non-Federal work experience may be creditable for leave purposes.
- Provide expert, highly specialized nursing care to patients with complex, high-acuity needs in ambulatory services; apply advanced clinical expertise and evidence-based practices.
- Performs comprehensive patient assessments, developing individualized plans of care, and coordinating interdisciplinary treatment to achieve continuity and efficiency of care.
- Independently develop, implement, and evaluate facility-level programs and local procedures supporting case management functions; interpret and adapt existing guidance to meet unique patient needs within the facility.
- Provide education to patients and members of the healthcare team; Identify potential gaps in care, recommend corrective actions, and monitor patient outcomes.
- Lead the development and implementation of quality improvement initiatives to optimize patient safety, clinical outcomes, and operational efficiency.
