The incumbent is responsible for providing clinical pharmaceutical care in clinical practice areas, assisting the Chief of Pharmacy with the day-to-day operations of Outpatient Clinical Pharmacy services, generating reports related to clinical pharmacy practice, supervising Specialty Clinical Pharmacy Practitioners (CPP), performing assigned staff evaluations and leading/monitoring Clinical Pharmacy quality improvement/assurance projects.

• VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Functions or Scope of Assigned Duties include but are not limited to the following: Using advanced knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics, the CPP serves as an authority on pharmaceutical care in specialized and routine areas of clinical pharmacy practice. The CPP serves as drug information resource, answering FS # 30818-F FLSA: Exempt BUS: 8888 Cybersecurity Code: 000 PDR: Low Risk; Tier 1 (April 2023 Matrix) PMIAA: 4 inquiries regarding therapeutics, drug interactions, dosing, side effects, formulary options, etc. The CPP is responsible for the evaluation, monitoring and medication management of Veterans referred by consult to the clinical pharmacy specialist for chronic disease state management. This will involve the design of a therapeutic regimen or plan, based on patient-specific information, which includes establishing a therapeutic endpoint, electing an appropriate drug and related therapy and establishing an appropriate dosage regimen. This includes the initiation, modification and discontinuation of appropriate pharmacotherapy and laboratory monitoring related to national guideline-based treatment of metal health chronic disease state medication management as consulted. The CPP obtains medication histories from patients and performs medication reconciliation at all transitions of care, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications. This may include adding non-VA medications or discontinuing duplicate medications or those the Veteran is not taking. Significant findings are summarized and documented in the medical record and communicated to other members of the health care team as appropriate. The CPP is actively involved in reviewing patient medication regimens for drug selection, dosing, contraindications, side effects, potential drug interactions, cost-effectiveness and therapeutic outcomes as appropriate. Findings and recommendations for alternatives to current treatment plan will be communicated to providers as appropriate. The CPP will monitor for and report medication errors, adverse drug reactions, allergies and patient compliance/adherence issues, documenting findings in accordance with facility policies and procedures. When assessing or providing patient care the CPP will consider the age, literacy level and learning barriers (e.g. low vision, hard of hearing, dementia, etc.) of the patient as it relates to his/her ability to understand and follow directions, provide objective self -assessment or any other pertinent factors which may affect the Veteran's overall treatment plan. The CPP reviews electronically submitted non-formulary drug requests utilizing National VA PBM Criteria for Use, completing review and documenting adjudication in Electronic Health Record (EHR) in a timely manner as determined by VISN and National PBM leadership. The CPP participates in Pharmacy Service and Pharmacy and Therapeutics Committee quality assurance/performance improvement activities, including MUE projects. The CPP serves on medical center and/or VISN committees as assigned. Manages the day-to-day operations of the RVAHCS Specialty Health Clinical Pharmacy services and serves as the immediate supervisor for the mental health, pain and substance use disorder, and CLC Clinical Pharmacy Practitioners (CPP). Resolves personnel matters dealing with employees and union representatives in regard to employee- management concerns. Approves or denies leave requests for CPPs based on workload needs. This includes the addition of new tours of duty as needed. Completes performance evaluations, recommendations for awards, and/or disciplinary actions for the CPPs and makes recommendations to the Associate Chief of Pharmacy, Clinical and Education, when appropriate. Participates in the development and revision of RVHACS Pharmacy Service policies, procedures, and training. Responds to elevated patient inquiries, as needed, and ensures that customer satisfaction is achieved. Conducts routine huddles for pharmacy staff as a means of communication and process improvement. Serves as a backup with respect to any duties normally performed by GS-13 Primary Care CPPs.
