This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. • VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU VA Careers - Licensed Clinical Social Worker: https://youtube.com/embed/U_xC25QsN0w Total Rewards of a Allied Health Professional This Supervisory Social Work position is in Social Work services at the VA Bedford Healthcare Systems, located at the Bedford, Mass location.The incumbent serves as the Post 9/11 Military-2-VA Case Management Program Manager as well as the Social Work Supervisor to Social Work staff embedded in the Patient Aligned Care Teams (PACT), the Weekend, Evening and Nights (WEN) coverage team and the Veterans Integration To Academic Leadership (VITAL) program. Duties include but are not limited to: Oversees transition activities and care coordination for OEF/OIF service members and Veterans. Explores and analyzes community resources including family-friendly delivery systems. Supervises Spcial Workers ambedded in Patient Aligned Care Teams (PACT), Weekend, Evening and Nights (WEN) coverage, and the Veterans Integration to Academic Leadership (VITAL) program. Develops systems to indentify and provide outreach to OEF/OIF patients, including those not currently seen by the facility. Serves as the facility point of contact for all inquiries regarding the seamless transition of OEF/OIF Veterans and Veterans seeking healthcare. Ensures OEF/OIF service members and Veterans are screened for care management needs and assigned appropriate case managers. Reseraches, explores and disseminates information on community resources to enhance continuity and quality of care for Veterans. Provides education and consultation to staff, Veterans, families, and community agancies about the VHA services, clinical guidelines, and Psychisocial issues. Monitors VHA performance measures related to OEF/OIF care and prepares required program reports for leadership andf oversight bodies. Advocates on behalf of Veterans to ensure timely acess to services and benefits, and acts as a liason between the VA, Veterans, and community partners. Ensures compliance with privacy, safety, infection control, and computer security policies, and fosters a safe work environment, and Assumes other related duties.
