VA Ann Arbor Healthcare System (VAAAHS) is seeking a Social Worker to work at their Jackson VA Community Based Outpatient Clinic (CBOC). The Social Worker is responsible for providing Outpatient Social Work services to Veterans who are receiving medical care predominately in the Patient Aligned Care Team (PACT) clinics, Specialty clinics, Emergency Department, and as a part of SW triage walk-in clinic located within the hospital setting.

• Total Rewards of a Allied Health Professional Major duties will include, but are not limited to: Provide social work services to PACT/ GM & Specialty clinics within VA Ann Arbor Healthcare Care System. Demonstrate a complete range of advanced social work methods, individually or with groups, and demonstrates a mature proficiency in psychosocial assessment, intervention, treatment planning, case management, crisis intervention, advocacy, and medical care coordination. Deliver primary care to Veteran patients using a patient centered, strengths-based approach to disease prevention, health promotion and improvement, care coordination, and disease management. Function as the subject matter expert with regards to VHA/VBN Federal, State and local government programs, benefits and services available to our Veteran population. Demonstrate a high level of skill in assessment, intervention and treatment plan development when addressing the comprehensive psychosocial needs of the Veterans He/she is assigned. Assess the patient in order to identifying emotional, social, physical, familial, economic instability, and vocational needs that may be causing distress and impacting the health condition or creating barriers to the patient receiving recommended care/treatment. Provide a thoughtful approach to assist the patient/ family to understand the contributing factors to the identified issue(s), discuss available short/long term solutions and/or resources, using a strengths-based empowerment model to formulate an appropriate patient centered intervention plan to reduce stressors, improve health and reach health related goals. Actively participate as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans/family/caregivers/Non-VA providers. Work Schedule: Monday - Friday, 08:00am - 4:30pm Recruitment/Relocation Incentives (Sign-on Bonus): May be authorized for a highly qualified candidate. Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
