The Remote Patient Monitoring - Home Health Care Coordinator, Registered Nurse (RN), is responsible for coordinating and managing remote patient monitoring services for patients in a home setting. This role focuses on improving patient outcomes through telehealth technology, chronic disease management, patient education, and timely clinical interventions.

• The Remote Patient Monitoring Home - Telehealth Care Coordinator, Registered Nurse (RN), performs duties that include but are not limited to the following: Collaborates with interdisciplinary teams to monitor patient health data, assess changes in condition, and promote continuity of care while supporting patients in maintaining independence and reducing avoidable hospitalizations. Coordinates care for a panel of patients to assure that care is timely, appropriate, of high quality, and cost effective. Work is performed using remote patient monitoring equipment. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Responsible for adhering to established policies, standards of care, and standards of practice and demonstrates leadership when providing and improving patient care. Ongoing patient monitoring and patient education are fundamental aspects of the RPM-HT program. Accurate and timely clinical documentation in the patient's medical record, using nationally mandated HT note title and templates, is required. Documentation in the HT program occurs at several times throughout the continuum of care, including during screening and eligibility, assessment, care, planning, intervention, evaluation, patient education, and transition management. Demonstrates integration of biopsychosocial concepts, cognitive skills, and technically competent practice while providing care to clients. Assesses, plans, implements, and evaluates care based on age-specific components. This position potentially requires flexibility in schedule and assignments.
