The Registered Nurse (RN) Home Telehealth Primary Care Specialty Medicine position is located onsite at the St Cloud VA Healthcare facility in St Cloud, MN. The RN is responsible for providing competent, evidence-based outpatient care to patients with oversight of licensed practical nurses/nursing assistants as appropriate to the setting. The RN integrates outpatient knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex situations.

• The Registered Nurse (RN) Home Telehealth Primary Care Specialty Medicine position is located onsite at the St Cloud VA Healthcare facility located in St Cloud, MN the selected applicant will report regularly for their tour of duty at the facility. Major duties include but are not limited to: Encouraging the appropriate use of health care services and strive to improve quality of care and maintain cost effectiveness on a case-by-case basis. Assist the patient in the safe transition of care to the next most appropriate level. Advocate for both the patient and facility to facilitate positive outcomes for the patient, the health care team, and the facility. Supports implementation of Patient Aligned Care Team (PACT) principles, participates in team huddles and implementation of redesign projects focused on improving the patient centered experience, improving patient' overall health, well-being, and satisfaction with care while meeting/exceeding goals of the VA organization. Demonstrates proficiency in medication management, including accurate preparation, administration, patient education, and documentation. Demonstrates skills/knowledge/ability to provide both face to face, video, and telephone care assessment/triage services. Applies nursing process, seeks resources, and identifies critical factors that may impact desired outcomes on discharge, post hospitalization recovery, or health maintenance/improvement. Sets clinical care goals, short and long term, in collaboration with patient, provider(s), including the Primary and Specialty Medicine team, the Mental Health Care team, Community Living Center, and family members. Maintains a working knowledge of resources available in the community. Assesses patients for use of appropriate home medical devices including telehealth equipment, follows procedures for ordering equipment, documents instruction/education in computerized patient record.
