The Practical Nurse in Community Care provides administrative support to clinicians, administrative and ancillary health care professionals through the Community Care Network of facilities within VA for purposes of coordinating non-VA medical care for Veterans across the inpatient and outpatient health care service, ordered and approved by their VA providers based on eligibility and deemed necessary for the best medical interest.

  • Total Rewards of a Allied Health Professional Major Duties: Provides administrative support to clinicians, administrative and ancillary health care professionals throughout the Community Care Network of facilities and within VA Acts as a liaison between the Veteran and the community providers/vendors with regard to non-VA Medical Care, VA clinical services and/or mental health services Generates authorizations and documents for pre-authorized medical care, while making eligibility determinations for the requested care based on the current VA policy Obtains requested records, X-rays, labs, and any pertinent information required to initiate and complete the ordered care requested in the CPRS consult Ensures all necessary components and documentation are distributed to appropriate services at the community facility where the service will be provided Prioritizes consults/orders requested based on priority status level, urgency, medical necessity based on VA policy and SOP Navigates the veteran to appropriate points of contact within the transportation service as needed, for warm hand off to the SME in transportation to address eligibility requirements and status Updates and verifies demographic information, verifying insurance information, data entry, administratively managing consultation requests and the electronic waiting list, mail correspondence and notifying the patient of the scheduled appointment with the outside providers Other duties as assigned Preferred Experience: Scheduling and Care Coordination experience. Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
