Registered Nurse - Outpatient Staff

This role will be based at the Cumberland CBOC in Cumberland, Maryland, serving the Martinsburg Veteran Affairs Medical Facility. The primary responsibility of the RN in this position is to deliver competent, evidence-based home-based primary care to patients. Additionally, the RN will oversee licensed vocational nurses and nursing assistants, ensuring high standards of care and supervision that are appropriate for the clinic setting.

Nurse I Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse The Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse I (RN) must demonstrate integration of biopsychosocial concepts, cognitive skills, and technically competent practice while providing care to clients. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Nurse II Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse The Nurse II Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs. Nurse III Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse The Nurse III Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Incumbent executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex HBPC care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.
