This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. • This position is located at the Spark M. Matsunaga VA Medical Center - Ambulatory Care Outpatient Clinic, in Honolulu, HI. The Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and evidence based approaches to management of complex primary care. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. The major duties include, but are not limited to: Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Provides urgent care to patients with acute and/or chronic medical, surgical, and psychiatric conditions. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented outcomes at the program or service level. Assesses the patient by collecting pertinent data or information, including the triage of patients based on the five-level triage system to prioritize patients that require more immediate care. Uses assessment data to determine diagnoses, problems, or other concerns. Identifies expected outcomes to develop a collaborative and individualized plan for each patient. Evaluates progress towards achieving the expected goal and outcomes and plans as necessary.
