This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

• VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Total Rewards: Total Rewards of a Allied Health Professional The Primary Care Mental Health Integration (PCMHI) Social Worker position is administratively aligned with the Mental Health Service and is assigned as a Collocated Collaborative Care (CCC) behavioral health consultant in Primary Care. Major duties include but are not limited to: Collaborating with Primary Care Providers and interdisciplinary PACT members to support effective and coordinated service delivery. Conducting focused functional assessments and providing time-limited, evidence-based individual or group interventions. Maintaining open-access appointments and facilitating same-day warm handoffs within PACT. Utilizing Measurement Based Care and stepped-care principles to appropriately match services with Veterans' clinical needs. Referring Veterans with more severe or complex needs to appropriate general or specialty mental health programs. Providing consultation and education to PACT members regarding behavioral health, health behavior, patient communication, motivation, and behavior change. Completing clear, concise, and timely clinical documentation to facilitate interdisciplinary collaboration. Participating in Social Work administrative activities, performance improvement initiatives, training, education, and program evaluation activities as assigned.
