Nurse (Case Management)

This position serves as the Nurse (Case Management) at the Jicarilla Service Unit, Dulce, NM. The incumbent provides well-established case management activities, coordinates care for patients with chronic and recurring health conditions and develops nursing skills in comprehensive assessment and care planning. This position reports to the Chief Nurse Executive or designee. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).

• Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Manages an assigned caseload of patients with chronic or recurring health conditions, conducts comprehensive nursing assessments, develops and adjusts individualized care plans, coordinates discharge planning and post-discharge services, monitors patient progress, and addresses barriers to care. Provides direct professional nursing care through in-person, telehealth, and community-based encounters; performs assessments and nursing interventions; administers medications and treatments; monitors treatment response; documents care; and supports patient flow and triage activities. Coordinates care across providers, disciplines, and healthcare settings; facilitates transitions of care; manages referrals, authorizations, and follow-up services; and participates in interdisciplinary rounds and care coordination conferences. Provides individualized education on disease processes, medications, treatment plans, self-management, and post-discharge requirements while using culturally appropriate health coaching and educational approaches. Collects and analyzes case management data, identifies care gaps and systemic barriers, recommends program improvements, contributes to policies and procedures, and serves as a case management resource to nursing staff and interdisciplinary team members.
