Social Worker

Citizens Memorial Home Health Services is recruiting a Behavioral Health Consultant to work with our Care Management team - someone who leads with skill, compassion, and a drive to positively impact every patient and family we serve. 

Position Details 

• Status: Full-time
• Work Shift: Days
• Hours: Monday-Friday, 8am-4:30pm

What Your Day Looks Like: Working with the Rural Health Clinic Medical Providers to identify patients with medical conditions that are newly diagnosed, not well controlled or affected by a possible underlying mental health issue or condition. The BHC helps the patient identify barriers and behaviors that lead to poor outcomes and, through a series of brief visits either in the clinic or by telephone, works to develop goals and strategies or behavioral modifications to help them reach their goals. 

As a Behavioral Health Consultant, you will be:  

• Performing screenings/evaluations of individuals for mental health and substance abuse disorders.

• Referring patients with underlying mental health conditions to an appropriate mental health provider.

• Providing behavioral supports to assist individuals in improving health status and managing chronic illnesses.

• Identifying problem behavior, discussing impact of intervention, and, with the patient’s input, deciding what behaviors to change to impact outcomes.

• Meeting regularly with the Patient Centered Medical Home team to plan care, discuss cases, and exchange appropriate information with team.

Qualifications

• Education: Bachelor's or Masters Degree in psychology or a related human-services discipline
• Skills/Experience: Related experience in healthcare, behavioral health, substance abuse counseling, or crisis assessment; management experience preferred
• Licensure/Certification: LPC, psychologist, LCSW preferred; LMSW, MSW, or BSW will be considered
• Physical Requirements: Prolonged periods of sitting to perform computer tasks
