JOB SUMMARY
To facilitate the referral/intake process for potential patients/clients, including receiving referrals; ensuring benefits are verified; gathering necessary medical information, orders, and face to face documentation ; and coordinating care with the patient/client, their family, attending physician, and the referral source for a smooth transition into agency care.
JOB REQUIREMENTS
Minimum Education
- Associate degree in Nursing
Minimum Work Experience
- 1 year of experience as a Licensed Vocational/Practical Nurse or Registered Nurse; home health experience preferred
Required Licenses/Certifications
- Registered Nurse or Licensed Vocational/Practical Nurse
- CPR
- Current Texas Driver’s License
Required Skills, Knowledge, and Abilities
- Ability to carry out obligations and duties of position Understand the physical, psychosocial, and spiritual concerns/problems of home health patients and families
- Knowledgeable of applicable policies and procedures
- Knowledgeable of Home Health Federal/State/and Joint Commission regulations
- Nursing skills as defined as generally accepted standards of practice
- Proven written and editing skills
- Strong computer skills
- Strong interpersonal skills
