About the Position: Serves as a Mental Health Care Facilitator that works with mental health providers to deliver evidence-based biopsychosocial assessment, treatment, prevention and management of mental health problems and biopsychosocially influenced health conditions for Military Health System beneficiaries. This is a Direct Hire Authority (DHA) solicitation utilizing the DHA for Certain Personnel of the DoD to recruit and appoint qualified candidates to positions in the competitive service.
- Perform clinical triaging in the primary care setting.
- Support primary care providers in treatment of patients with mental health conditions.
- Facilitate continuity of care through a multidisciplinary approach.
- Provide patient education as an integral part of the services offered.
- Assist patients with treatment plan adherence.
