Nurse - Domiciliary Mental Health

The Domiciliary Mental Health Registered Nurse provides patient-centered, recovery-oriented nursing care to Veterans with mental health and/or substance use disorders. The RN coordinates care, provides patient education, promotes safety, and collaborates with the interdisciplinary treatment team to support positive patient outcomes throughout the continuum of care.

• The Domiciliary Mental Health Registered Nurse provides competent, evidence-based nursing care to patients with mental health and/or substance use disorders. Duties include, but are not limited to: Provides patient-centered and recovery-oriented nursing care to Veterans with mental health and/or substance use disorders. Assesses patient needs and develops, implements, and evaluates nursing plans of care in accordance with established policies and clinical standards. Coordinates patient care and collaborates with members of the mental health interdisciplinary treatment team. Provides patient education related to health conditions, medications, treatment plans, self-management, and recovery. Facilitates shift reports, warm hand-offs, and nurse-led psychoeducational groups. Uses therapeutic communication, recovery-oriented language, and appropriate de-escalation techniques. Performs and documents nursing assessments, risk screenings, safety rounds, and environment-of-care rounds. Provides oversight of the patient safe medication management program and promotes safe medication practices. Identifies and responds to psychiatric and medical care needs and emergencies in accordance with established procedures. Reports and assists with the resolution of patient safety concerns and maintains professional boundaries to protect patient vulnerabilities. Participates in quality improvement activities and initiatives designed to improve patient care and program outcomes. Supports nursing colleagues and other healthcare professionals through collaboration, knowledge sharing, and evidence-based practice. Promotes a safe, supportive environment that contributes to quality patient care and the professional development of healthcare staff.
