About the Role

As a Hospice Social Worker (MSW), you will be an essential member of our interdisciplinary care team, providing comprehensive psychosocial support to patients and families navigating end-of-life care.

You will help patients and families address the emotional, social, practical, and family-related challenges that can arise throughout the hospice journey. This includes providing counseling and supportive interventions, assisting with advance care planning, connecting families with community resources, and providing grief and bereavement support.

We are looking for a compassionate, dependable, and solutions-oriented social worker who understands the complexities of grief, loss, and end-of-life care and is committed to providing dignified, person-centered support.

This position requires the ability to travel throughout the assigned service area and may include participation in an on-call rotation as required.

What You'll Do

Responsibilities may include, but are not limited to:

💙 Provide Comprehensive Psychosocial Support

• Conduct thorough psychosocial assessments of hospice patients and their families within required timeframes.
• Assess patients' and families' coping mechanisms, support systems, strengths, preferences, and available resources.
• Identify psychosocial needs and develop individualized care plans to address those needs.
• Provide compassionate, culturally sensitive support to patients and families throughout the end-of-life journey.

🤝 Provide Counseling & Supportive Interventions

• Provide individual and family counseling related to grief, loss, anticipatory mourning, end-of-life decision-making, and other psychosocial concerns.
• Provide crisis intervention, stress management, coping skills, and emotional support as appropriate.
• Help patients and families navigate emotional, interpersonal, and practical challenges associated with serious illness and end-of-life care.
• Educate patients and families about the dying process and provide emotional preparation for anticipated loss.
• Make visits as scheduled and participate in the on-call rotation as required.

📋 Support Advance Care Planning & Decision-Making

• Facilitate conversations regarding advance care planning, healthcare directives, durable power of attorney for healthcare, and goals of care.
• Assist patients and families in making informed decisions regarding hospice enrollment and end-of-life care preferences.
• Advocate for patients' wishes, values, and autonomy throughout the hospice experience.
• Support patients and families in navigating complex end-of-life decisions with compassion and respect.

🌿 Connect Patients & Families with Resources

• Connect patients and families with appropriate community resources and support services.
• Assist with referrals for financial assistance, caregiver support, home health services, support groups, bereavement counseling, and other community-based resources.
• Collaborate with interdisciplinary team members to coordinate referrals and support continuity of care.
• Help patients and families access resources that promote comfort, stability, and quality of life.

🕊️ Provide Bereavement Support

• Provide anticipatory grief support to patients and families facing the impending loss of a loved one.
• Follow up with bereaved family members and caregivers following the patient's death.
• Provide ongoing emotional support, counseling, education, and referrals as appropriate.
• Help families navigate the grieving process and connect them with additional bereavement resources when needed.

📝 Document & Coordinate Care

• Complete all required documentation and paperwork in accordance with hospice policies and procedures.
• Follow the physician's Plan of Care for Medical Social Services.
• Maintain accurate and timely documentation of assessments, visits, interventions, care plans, and follow-up.
• Communicate changes in psychosocial needs and patient/family concerns to the interdisciplinary team.
• Collaborate with physicians, nurses, chaplains, aides, and other members of the care team to support comprehensive patient care.

Qualifications

The ideal candidate will demonstrate:

• Master's degree in Social Work (MSW) from an accredited program.
• Minimum of one (1) year of experience in social work, counseling, or a related healthcare field.
• Current automobile insurance and a valid driver's license.
• Reliable transportation and the ability to travel throughout the assigned service area.
• Strong understanding of grief and loss, psychosocial dynamics, and family systems.
• Excellent communication, interpersonal, and counseling skills.
• Ability to establish rapport and build trust with patients and families facing end-of-life issues.
• Commitment to the principles of hospice and palliative care.

Preferred Qualifications

• Current, unrestricted LCSW or LMSW license in the state of practice.
• Previous hospice or palliative care experience.
• Experience providing counseling or psychosocial support to patients and families facing serious illness or end-of-life care.
• Knowledge of local community resources and support services.
• Experience coordinating referrals and collaborating with interdisciplinary healthcare teams.
• Strong understanding of bereavement and the unique needs of individuals and families experiencing loss.
• Compassionate, patient-first mindset with a commitment to dignified, person-centered care.
