This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. • Atlanta VA Medical Center is actively seeking a Registered Nurse for Outpatient Staff in Specialty Care Service Line. General Description of Assigned Duties: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education , self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team . Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. Duties include but not limited to: Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Serves as a team leader, providing clinical guidance to licensed practical nurses and health care technicians. Cross-trained to provide clinical triage support for all the unit's clinics (i.e. - Dermatology, Sleep, Rheumatology), includes Face-to-Face, VVC, and Telephone Visits. Documents assessments and interventions by policy and practice standards. Assists with medication management, including administration and pyxis inventory. Participates in unit-level quality improvement activities such as projects, audits, etc. Complete daily, weekly, and monthly assigned monitors. Mentors/precepts new staff and nursing students. Crisis intervention for unique population. Provide care coordination to improve continuity, access, and quality of care. Provide patient/family health education with a focus on self-management, prevention, and wellness, based on the patient's personalized health plan/goals. Executes nurse driven clinics, including facilitating group visits, for appropriate patient populations to support positive health outcomes for Veterans. Provide patient education and follow-up related sleep study results per Provider interpretations (Sleep Clinic). Assists with setting up and providing surgical assistance for Surgery and Biopsy (Dermatology) clinics. Completes post-surgical phone calls (Dermatology) within 2 business days, and documents results in patient's medical record. Other duties as assigned.
