About the Role

As an On-Call Registered Nurse (RN), you’ll be an important part of that experience. You’ll provide compassionate, responsive care to patients and families when they need support after hours, helping ensure they feel heard, cared for, and never alone.

What You'll Do

Responsibilities may include, but are not limited to:

🩺 Provide Compassionate Clinical Care

• Complete thorough assessments of hospice patients and respond to changes in condition, symptoms, and care needs.
• Provide timely clinical interventions to promote comfort, dignity, and quality of life.
• Develop and adjust individualized care plans in collaboration with patients, families, and the interdisciplinary team.
• Use your clinical judgment to address after-hours needs and provide appropriate guidance and support.

💊 Help Patients Feel Comfortable & Supported

• Assess and manage pain and other symptoms using appropriate pharmacological and non-pharmacological interventions.
• Safely administer medications and monitor patients for effectiveness and potential adverse effects.
• Educate patients and families on medications, symptom management, and comfort measures.
• Help families understand what to expect and provide reassurance during challenging moments.

💙 Be a Source of Support for Families

• Provide compassionate emotional support to patients and loved ones facing end-of-life care.
• Listen to concerns, answer questions, and help families feel confident in the care being provided.
• Support conversations surrounding goals of care, advance care planning, and end-of-life wishes.
• Approach every interaction with empathy, kindness, patience, and respect.

🤝 Work Together as a Team

• Communicate closely with physicians, hospice leadership, aides, social workers, chaplains, and other members of the interdisciplinary team.
• Keep the care team informed of changes in patient condition and emerging needs.
• Coordinate services, referrals, and transitions of care when needed.
• Help ensure each patient receives seamless, personalized care throughout their hospice journey.

📝 Document with Care & Accuracy

• Maintain timely and accurate documentation in the electronic health record (EHR).
• Document assessments, interventions, communications, and outcomes in accordance with organizational and regulatory requirements.
• Follow Medicare Conditions of Participation, state regulations, accreditation standards, and hospice policies.
• Participate in quality improvement efforts that help us continually strengthen the care we provide.

Qualifications

The ideal candidate will demonstrate:

• Current Registered Nurse (RN) license in the state of practice
• At least one year of nursing experience
• Hospice, palliative care, home health, or related experience is preferred
• Strong clinical assessment and critical-thinking skills
• Ability to develop and follow individualized plans of care
• Excellent communication and interpersonal skills
• A compassionate, patient-centered approach to end-of-life care
• Knowledge of hospice philosophy, palliative care, and end-of-life issues
• Ability to work independently while remaining connected to the interdisciplinary team
• Comfort using electronic health records and completing clinical documentation
• Reliable transportation and willingness to travel throughout the assigned service area
