This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Duties include but not limited to: Practices as a licensed independent Practitioner, utilizing face to face visits, telephone visits, and VA video connect visits (VVC). The incumbent may be assigned to any of the outpatient clinics within the catchment area of the VA Southern Oregon Rehabilitation Center & Clinics. Responsible for providing primary care to an established panel of male and female Veteran patients. Functions as a member of a patient aligned care team (PACT) consisting of a physician, an RN Care Manager, a clinical associate (LPN or Health Tech/Medical Assistant/Intermediate Care Tech.), and a clerical associate (Medical Support Assistant). Provides comprehensive interdisciplinary care in coordination with extended PACT support to include a Clinical Pharmacist, PACT Social Worker, Outpatient Dietitian, Primary Care Mental Health Integration Psychologist, and the Health Promotion Disease Prevention Program Manager. Consists of Veteran patients aged adult through geriatric. Work Schedule: Monday-Friday, 8:00 am-4:30 pm
