Social Worker (Outpatient Mental Health)

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

• Provides education on advance directives and advanced care planning. Acts as an advocate with appropriate VA and community service providers/agencies when it serves the best interest of the Veteran and family members/caregiver. Maintains a current network of internal and external resources to educate the Veteran and/or family members/caregivers and assist with the appropriate referrals. Participates in the psychosocial therapeutic treatment of Veterans through collaboration with Veterans and their family as well as interdisciplinary treatment team members in the development and implementation of a recovery-oriented treatment plan in facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence that is possible and practical. Assesses the psychosocial functioning and needs of Veterans and their family members identifying the Veteran's strengths, weaknesses, coping skills and psychosocial acuity, in collaboration with the Veteran, family, and interdisciplinary treatment teams. Provides ongoing treatment and documentation per VA Guidelines and facility policy. Completes consults from interdisciplinary teams and other social workers on complex difficult cases using advanced practice skills and expertise. Interviews and conducts psychosocial assessments to establish veteran's treatment needs, presenting problems, and provide a clinical impression. Assesses high risk factors, acuity and need for services. Collaborates with interdisciplinary teams such as OIF/OEF/OND case manager, substance abuse staff, Psychiatry (inpatient/outpatient), Homeless Program, Physicians and Vet Centers. Acts as an advocate when appropriate and feasible while encouraging the veteran and family to advocate on their own behalf thus fostering a sense of independence and empowerment. Makes rapid assessments and develops crisis management plans for admission to mental health inpatient programs. Provides psychoeducation and clinical feedback regarding symptoms of the mental health condition to patients served. Possess working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, common medications and their effects/side effects, and medical terminology.
